Minute by minute, from the first cleanse to the SPF

It opens with about 5 minutes of looking. Your esthetician cleans the skin, gets it under good light, and says out loud what is there: open comedones across the nose, closed ones along the jaw, two or three inflamed papules, and often the sebaceous filaments you have been calling blackheads since high school. Those are three different things, and they do not come out the same way.

After that the order usually holds. Double cleanse, 5 to 8 minutes of steam, extractions, a clay or enzyme step, then hydration and SPF. What changes is how long each part gets. On congested but calm skin, extractions can take the full 15 minutes. On angry skin they take 4 or 5, and the mask does most of the work instead.

One thing worth saying out loud in the room: tell your esthetician the moment something hurts. There is no prize for sitting through a hard extraction with your jaw clenched. A spot that fights back today usually lifts out easily 3 weeks later, once the skin around it has calmed down, and nothing is lost by waiting for it.

Why extractions usually stop around 10 to 15 minutes

There is a point where extraction stops helping. Skin goes red, the follicle gets bruised, and the dark mark left behind outlasts the blackhead by weeks. So the work usually stays inside a window of roughly 10 to 15 minutes, and whatever is left over waits for the next visit, about 3 weeks out.

The last 10 minutes are not filler

Breakout-prone skin usually arrives over-treated: a foaming wash, 2 competing actives, a spot treatment and an alcohol toner stacked on top of one another. A facial that ends the same way wastes the hour. The closing steps put water and lipids back over the last 10 minutes or so, so a stripped barrier is not what greets you on day 4.

Where a licensed esthetician stops and a dermatologist starts

This part usually gets skipped at booking, so we will do it here. Extractions empty the top of a follicle. They do nothing for inflammation sitting deeper than hands-on work can reach. Breakouts that are deep, painful, slow to surface and slow to fade are a different problem, and that problem belongs in a doctor's office.

Plenty of our regulars run both tracks at once: a prescription from their dermatologist, and an acne facial every 3 or 4 weeks to keep surface congestion down while the medication does the actual work. Bring the name of anything you are taking, including anything started in the last 30 days. It changes what we can safely do that day.

What extractions can clear

Open comedones, closed comedones, sebaceous filaments, congestion that builds under heavy sunscreen or long-wear makeup, and the rough texture that rides along with all of it. That difference is usually visible the same day, once the first flush of pink settles over 2 to 6 hours.

What we hand back to a doctor

Cystic and nodular acne. Breakouts that scar. A sudden severe flare, anything painful under the surface, and rosacea or perioral dermatitis, which get mistaken for acne constantly. We will still see you for a gentler session, but we skip extractions over any active, inflamed lesion, every time.

Teenage skin, and why lighter beats harder

Adolescent skin makes more oil than it will at 30, and it turns over faster. That sounds like an advantage, and half the time it is: a 15 year old's post-extraction pink often clears in 2 hours rather than 6. The other half is the problem. Fast healing hides damage, so an aggressive session on a 16 year old feels fine on the drive home and shows up as irritation on day 3.

So the session runs lighter, not harder. Less pressure, an extraction window closer to 8 minutes than 15, nothing aggressive. A 14 year old who scrubs the barrier off spends 2 or 3 months rebuilding it, and the breakouts get worse across those months, not better. Most of what we do in the room for a teenager is subtraction: take things away, then see what the skin does over the next 4 weeks.

Homecare beats the appointment

The blunt version: one facial does less for a 15 year old than washing twice a day, changing the pillowcase every 3 days, and leaving the mirror alone. Teens go home with 3 or 4 habits, not a 7 step routine nobody keeps past week 2.

Helmets, chin straps and August in Knoxville

Chin straps, shoulder pads and 90 degree two-a-days push congestion along the jaw and hairline every fall. If your teen plays a fall sport, book 2 weeks before practice starts and once more mid-season, instead of trying to undo 3 months of it in November.

Deep cleaning, acne facial, or neither?

People ask for the deep cleaning emphasis when nothing is actively breaking out but the skin feels rough, the pores look bigger than they did 6 months ago, and makeup starts sitting badly by 2 in the afternoon. Same customized facial, tipped toward extraction and away from calming.

Less time extracting, more anti-inflammatory work, and your esthetician will decline to squeeze an inflamed papule no matter how much you want it gone by Friday. Neither one is a separate service on the menu. Both fill the same 50 to 60 minute slot.

• Ask for the deep cleaning emphasis if your skin is congested but calm, with no new breakouts in 2 weeks.
• Ask for an acne facial when whiteheads or papules are active and you would rather not be left with a mark for 3 months.
• Ask for a calm session with no extractions if the skin is peeling or sore right now, then rebook the extraction work 2 or 3 weeks out.
• Ask for hydration or the anti-aging facial if congestion is not the complaint, since dry and oily skin want opposite things.
• Call a dermatologist first if breakouts are cystic, painful, or no better after 6 months of trying.

Showing up ready: what actually helps before a facial

The most useful thing you can bring is an honest account of what is already going on your face. Serums, prescriptions, anything a dermatologist started you on, anything you bought 2 weeks ago because a video said to. That is what the intake form is for. Your esthetician reads it before the first cleanse and either changes the plan for that day or moves you to a date that suits the skin better.

A few practical things. Come with your hair off your face if you can, since the headband goes on in the first 2 minutes. Skip a fresh spray tan, because the first cleanse will take some of it off. If you got sunburned at the lake over the weekend, call and move the appointment, since working over a burn is not something we will do. And eat beforehand: 60 minutes flat on your back on an empty stomach is a long 60 minutes.

• Come with a bare face if you can, or budget an extra 3 or 4 minutes at the start for makeup removal.
• The usual spacing is 48 hours clear of facial waxing or threading on either side of the facial.
• Book a morning slot if the pink worries you, since a 10 am appointment leaves it 8 hours to fade before dinner.
• Skip the second coffee and drink water instead, because lying still for 50 to 60 minutes is easier without it.
• Move the appointment if a cold sore is active, because we will not work over one.
• Arrive 10 minutes early the first time, since the intake form takes about 5 minutes.

Pink for an afternoon: what the next 6 weeks look like

Extractions are controlled trauma. Small, deliberate, but still trauma. Skin answers by going pink and slightly warm, and that is the expected response rather than a sign something went wrong. It usually settles in 2 to 6 hours, occasionally by the next morning if a lot of work happened around the nose and chin.

The more useful question is what the next couple of months look like. One acne facial is a reset. A run of 4 to 6, spaced about 3 weeks apart, is where people tend to notice a change that holds, because each session starts from a cleaner baseline than the one before it. After that, roughly every 6 weeks is enough for most.

• First 2 hours: leave the skin alone, skip makeup, and let the SPF sit where it was put.
• Hours 2 to 6: the pink fades, and a spot or two may scab over, which is not an invitation to pick.
• First 24 hours: nothing that makes you sweat, so no gym, no hot yoga and no 30 minute sauna session.
• Day 2 to 3: a few spots already forming underneath can surface early, and that usually settles within a week.
• Day 3 onward: SPF 30 or higher every single morning, since freshly extracted skin marks easily in Tennessee sun.
• Week 3: rebook while you can still see the result, since the interval is what compounds, not the single visit.

Will an acne facial clear my acne?

Not on its own, and we would rather say so at booking than after. An acne facial clears surface congestion: blackheads, closed comedones, sebaceous filaments. It does not touch the hormones or bacteria underneath, and acne is a medical condition. A run of 4 to 6 sessions about 3 weeks apart, plus steady homecare, is what people actually notice. Cystic acne needs a dermatologist.

How many visits before I see a difference?

Something the same day, usually fewer visible blackheads and smoother texture once the pink settles. But a single visit resets things rather than settling them. A skin cell needs something like 28 days to reach the surface at 20, and longer at 50, so visits about 3 weeks apart land while the last one still counts. Most people do 4 to 6, then hold at every 6 weeks.

Do the extractions hurt?

There is real pressure, and around the nose and chin it can sting for a second or two. After 5 to 8 minutes of steam the plug is soft and most of them lift out with very little force. Say something the moment it crosses your line. One spot is not worth 3 weeks of a dark mark, and it will still be there next visit.

My teenager wants one. What should I know?

Expect a lighter session than an adult gets. Teenage skin makes more oil and heals faster, so the temptation is to go harder, and harder is what leaves a 15 year old with irritation for 2 weeks. Shorter extraction pass, no aggressive acids. Most of the value goes home with them: a plain wash morning and night, a fresh pillowcase 2 or 3 times a week, sunscreen, and hands off the face. Give that 8 weeks before deciding it is not working.

Can I book one the day before an event?

Please do not. Skin stays pink for 2 to 6 hours, and for some people a few spots surface over the following 2 or 3 days. For a wedding or senior photos, come in 10 to 14 days ahead, or start a series 6 weeks out. Inside 48 hours of the event, ask instead for a calm session with hydration and no extractions.

Is the Glo2Facial a better fit than an acne facial?

Different tools, same 50 to 60 minute slot. The Glo2Facial is our most-requested facial: oxygen infusion across the whole face, good for dull, congested skin. This session spends its 10 to 15 minutes of extraction on specific plugs, by hand, under a lamp. If the complaint is flat, rough texture, ask about the Glo2Facial. Deep cleaning or acne facial, which should I ask for?

If nothing is actively breaking out and the skin just feels congested, ask for the deep cleaning emphasis: more time on extraction, a firmer clay step. With active whiteheads or papules, say that at booking and the session goes anti-inflammatory instead. Neither is a separate service. Both are a customized facial in the same 50 to 60 minute slot.

Will it make me break out worse?

Sometimes, briefly. Around day 2 or 3 a few spots that were already forming can surface sooner than they otherwise would, and that generally settles inside a week. If you are still breaking out at day 10, or the breakouts show up somewhere we never touched, that is not the facial and we want to hear about it.
